Here you and your student will find information about healthy habits and resources that are available to help meet your student's health needs.
If you have any questions or concerns, you can call, email, or complete a nurse referral form (located below) to contact your child's school nurse.

Announcements
Kindergarten: All students are required to have a dental and physical exam on file with the school nurse. Please have your doctor or dentist fill out the form that can be located in the Forms section. If you are unable to print the form, please contact your child's school nurse. Return these forms to the school nurse as soon as possible. This helps to ensure that we are providing the best care to your student.
3rd Grade: All students are required to have a dental exam on file. Please have your dentist fill out the dental form located in the forms section. If you are unable to print the form, please contact your child's school nurse. Return the form to the school nurse as soon as possible.
Change of clothing: A "just in case," weather appropriate change of clothing should be kept in your child's book bag or locker. Accidents happen and in order for your student to not miss instructional time, a change of clothing being readily available avoids unnecessary missed class time. Thank you!
Immunizations: Students are required by the State of Pennsylvania to have certain immunizations given at recommended intervals. If you are missing any of these or we have questions, your school Nurse will contact you. Please be sure that all your contact information is kept up to date in skyward.
Health Information: In order to take the best care of your student, please let us know if their health changes, such as new allergies, medications, or any other concerns.
