Life Event Notice for Current Staff
When you experience a qualifying life event, you are eligible for a 30-day window to make changes to your benefits that are directly impacted. This allows you to update your coverage to reflect significant life changes.
Qualifying Life Events include:
Birth or adoption of a child
Loss of other health coverage
Loss of Life of a dependent
Change in marital status (e.g., marriage or divorce)
Other qualifying events as defined by your benefits plan
Important:
Proof of the life event (e.g., letter form HR (loss of coverage), proof of life or adoption certificate (birth), etc. ) is required to process your request.
If you are adding dependents, please also provide proof of relationship (e.g., birth certificate, marriage certificate, or legal documentation).
Social Security cards should be presented in person to be copied and kept on file for confirmation.
Please complete the form and submit any necessary supporting documents within the 30-day window. If you have any questions, contact HR for assistance.
