Vaccination Clinics will be brought to you from the Little Clinic by Kroger Health in partnership with Wellness Maricopa. The Centers for Disease Control and Prevention (CDC) recommends any available age-appropriate (as listed below) vaccine product for people aged 6 months and older. If you have specific questions about the appropriateness of a vaccine, please consult your PCP ahead of your vaccination appointment.
Vaccination clinics are available to all employees and their families. Your picture ID and insurance card will be required at the time of service; premade copies will help alleviate delays in the process. Credit card payments will be accepted for part-time staff, adjuncts, and student employees without insurance. Walk-ins will also be welcome, provided there are still time slots available.
Kroger Health is offering a $5 off coupon for the flu vaccine only, a $20 off coupon for Flu + another vaccine. Coupons can be used at any Fry’s store here in Phoenix; one coupon per transaction.
Additional Vaccination Clinics will be available on all other campuses not participating in Regional Expos, along with Wellness Stations and OE Benefit Stations where one of our expert benefit team members can answer Open Enrollment questions.
Mesa Community College will also host a Biometric Screening for those living and working in the East Valley.

Biometric Screenings are part of the Wellness Incentive Credits (WIC) redesign project interim solution to begin the 2025 plan year to ensure the continuity of service of Health Risk Credits.
Redesign of the health risk credit of $20/month. Employees enrolled in the MCCCD medical plan must complete an annual Primary Care Physician (PCP) preventive health visit or biometric screen.
Complete a biometric screening at one of the 2025 Open Enrollment Events listed above, OR
Complete a biometric screening at any participating LabCorp testing facility by November 22, 2024, OR
Complete an annual preventive visit with their Primary Care Physician (PCP), also known as an annual wellness visit or annual physical, on or between November 1, 2023, and November 22, 2024.
If you have visited your PCP for a preventive care visit between November 1, 2023, and November 22, 2024, there is nothing more you need to do. Your name will be on a list of completed screenings/annual visits provided by Banner|Aetna.
If you have seen your PCP for a preventive care visit and are not on the list provided by Banner|Aenta, we will contact you. At that time, you will be asked to complete an attestation form indicating that you did indeed have your visit for preventive health within the required time window.
If you are currently in the Virta program for type II diabetes reversal, you do not have to complete a biometric screen to be eligible for your health risk credit.
Biometric screens help you identify and monitor (over time) any concerning health conditions and include assessments of Cholesterol, Blood Sugar, Blood Pressure, Height, Weight, BMI, and Waist Circumference. They promote engagement in preventive health so you can make educated decisions about your healthcare.
Biometric screenings will be provided by LabCorp this year and will be available at designated open enrollment events, including those at Phoenix College, Scottsdale, Rio Salado, Glendale, Mesa, and Paradise Valley. The screens will not require you to be fasted and will involve a fingerstick. A fingerstick is a minimally invasive procedure that involves drawing a drop or two of blood from a fingertip using a lancet (small needle).
In order to sign up for an appointment, you will need to first create an account with the LabCorp WellConnect Plus system. The system will be available beginning October 10, 2024. You will need the following information to create your account.

Please make sure your First & Last Name is the same as listed on the Banner|Aetna insurance card
Birthdate - MM/DD/YYYY
Employee - Member ID Number printed on your Banner|Aetna insurance card without the leading "0"
This is the same as your Employee ID number. If you do not have your insurance card available, you can find your Employee ID number by:
Logging into the Human Capital Management (HCM) system as linked here. This may trigger the two-factor authentication process.
From your Employee Self Service page select the “Primary Job Details” tile.
Once opened the employee ID is located in the upper left-hand side of the page.
Select the "Continue" button, complete the rest of the field information, and then select the "Register" button.
Once your account is created, you will follow the steps listed in the LabCorp How to Schedule On-Site Campus Biometric Screening instruction booklet.
Aetna Covered Spouses/Domestic Partners
Under the "Aetna Covered Spouses/Dependents Register Here" section, select "Create An Account"
Use the Visitor Access Code = MCCD24
Please make sure your First & Last Name is the same as listed on the Banner|Aetna insurance card
Complete the rest of the field information and then select the "Register" button.
Once your account is created, you will follow the steps listed in the LabCorp How to Schedule On-Site Campus Biometric Screening instruction booklet.
If you are unable to schedule a biometric screening during a Campus Event, you can schedule a Patient Service Center screening at any LabCorp Facility near you by November 22, 2024. Follow the same instructions above to create an account through the LabCorp WellConnect Plus system. Then, use the How to Schedule an Off-site LabCorp Patient Service Center Appointment instruction booklet to set up your appointment.
