REQUIREMENTS:
Accomplished CBPSME Application Form (please input 'NA' in each category if not applicable)
Birth Certificate (Photocopy)
Medical Certificate (Form 3)
Research Plans (Annex A)
Post-Scholarship Plan (Annex B)
Approved Program of Study (POS) by the Department Graduate Coordinator
Certificate of Employment
Certified True Copy of the Official Transcript of Records (TOR)
Certified True Copy of Service Record (for applicant who is employed before)
Recommendation letter from two (2) former professors in college for MS; in Master's program for PhD
Ask your professors to upload the recommendation letters here.
Endorsement from the Sending Institution (if employed)
Notice of Admission to Graduate Program
Valid NBI clearance
ADDITIONAL REQUIREMENTS:
For DepEd employees:
Eligibility to Go on Study Leave on Official Time (Form 2A)
Letter of Intent for approval of the Head of Office
Substitution Plan from the School Head prior to the approval of the scholarship of each teaching personnel
For Private Schools and HEI employees:
Permit to Go on Study Leave (Form 2B)
For the Currently Unemployed:
Notarized Affidavit of Unemployment
For JLSS-RA 10612 scholars:
Service Obligation Clearance
ADDITIONAL REQUIREMENTS FOR LATERAL ENTRANT:
Certification from the university indicating the following:
number of gradate units required in the program
number of graduate units already earned with corresponding grades
Approval Sheet of Thesis/Dissertation Research Topic (for applicants currently on thesis/dissertation stage)
WHERE TO SUBMIT:
Initially send the soft copy of the application form and all supporting documents via email to: cbpsme@dlsu.edu.ph.
Kindly put in the Subject Header line: 2025-2 CBPSME Application-Last Name
All files must be in a PDF.
Once submitted, you will be notified on how to submit the hard copy documents.
REMINDER:
Both soft and hard copies of the accomplished application form and supporting documents must be submitted on or before the deadline.
If all the abovementioned requirements are not met, the application cannot be processed.
DEADLINE OF SCHOLARSHIP APPLICATION (every term)
Term 1 AY 2025-2026 : July 31, 2025
Term 2 AY 2025-2026 : November 14, 2025
Term 3 AY 2025-2026 : March 14, 2025

Admission for Term 2 AY 2025-2026 is from September 8 to October 11, 2025.
A different set of application documents must be submitted to the Admission Office of the University.
You can apply for both the Graduate Program and the scholarship simultaneously.
