Rental Contract

School owned instruments have been played by previous students and are well played. If you do not want to use a mouthpiece someone else has used it is recommended that you purchase your own mouthpiece. 

Mouthpieces range in cost from $30 - $60. If you have made arrangements with Mrs. Sczepanski to rent an instrument from the school please fill out this form and return to Mrs. Sczepanski with your payment.
