Health Office
Below is a list of the documentation required to be up-to-date in our health office and for your child to attend school. Printable forms can be found by clicking here and should be submitted to the Health office prior to August 16, 2021. Students that do not have required information on file will be excluded from school beginning October 15th until it is provided.
Freshmen
Health Form that includes physical exam and vaccination information (completed less than 12 months ago)
Please note - A sports physical does not meet the requirements for the 9th grade physical exam.
Dental Exam (completed less than 18 months ago)
Polio Vaccine (must have been given on or after the 4th birthday)
Tdap Vaccine (was required for all students grades 6 through 12)
MMR (measles, mumps and rubella) vaccine (2 doses)
Hepatitis B Vaccine (series of 3 injections)
Chickenpox (Varicella) Vaccine (2 injections OR proof of having had Chickenpox)
Seniors
2nd Dose of Meningococcal Conjugate Vaccine (Meningitis)
Students new to Illinois
Physical Exam
Dental Exam
Vision Screening
All Vaccinations
Sophomores, Juniors, or Seniors Participating in Sports
Illinois Sports Physical (Complete physical can also be used) **Please note the sports physical is only for grades 10-12.

Health Conditions
If your child has a health condition that may require care, please report this information directly to the Health Office. Authorization of all medications brought on school grounds must be verified by the Prescriber, the Parent/Guardian, and the School Nurse. Requirements, Authorization Forms, Action Plan Templates for Common Conditions, and Contact Information can be found in the Health Office section of the district website.
Completed forms can be brought to your child’s school or faxed to 866-904-9953.
