Registration Documents

This is a list of documents that may need to be returned to the school based on student grade level.

The Health Form and Military Privacy Form can be returned electronically to OLR@Dist156.org, mailed to the appropriate school or dropped off during school business hours.

State of Illinois Health Form - Form must be completed by a doctor and returned to the school for all incoming freshmen and new students.

Military Recruitment Privacy Form - This optional form should be returned by 10th grade students ONLY if parent/student wish that the student's information (name, grade level, address, and phone number) NOT be released to the military. (Spanish Version)

Chromebook Loan Agreement

INCOMING 9th GRADE AND NEW STUDENTS. A completed Chromebook Loan Agreement (English - Spanish) must be on file for all incoming 9th graders and new students prior to receiving a Chromebook.

If your child is taking summer school this form must turned in on the Chromebook pick up day for summer school students. Download and bring the completed form with you to pick-up.

If your child will be receiving their Chromebook on the material pick-up days in August, this form can be returned when they visit the school to obtain the computer.

Parking (Upper Campus)

Students must be registered and all fees must be paid in order to receive a parking pass. Fees include current fees and past due balances.

Parking passes for the 2021-2022 school year will be allocated to students using a new process. Please read this FAQ document (click link for access the document) to learn about more about this process and apply for a parking permit if eligible.

**Code of Conduct and Acceptable Use Policy - The Code of Conduct and Acceptable Use Policy will be completed as a part of the online registration process.
