The following Practice Cases are examples for workflows for Case Management, ICCM, CBMNC and Family Planning. This is intended for self-study on how to use the MVTK application. Refer to the DEMO section, for additional video examples.

Case Management

Register Household: Banda Household

- Go to Village Clinic - Register New Household- Household name = Banda- Traditional Authority = Chiseka- Village Name = Bitchayi- House address = Left at the church- Are you currently at the house? Yes- Capture GPS - Press “Record Location”- If it takes too long, press “cancel”, then press blue arrow for next question- Phone number = 2658855458- Press Finish to submit the form

Register Client: Child under 5, named Matthew Banda

- Client first name = Matthew- Client last name = Banda- Gender = Male- Choose Birth Date- 21 July 2016 (adjust the date accordingly for the child to be 18 months old)- “Matthew is 18 months old”- Who is Matthew’s caregiver? Mother (Name: Mary Banda)- Is Matthew on ARV treatment = Yes- History of TB contact = No- TB Status = Tested negative- Press Finish to submit the form
If you select Village Clinic - Banda household - Matthew Banda - Continue - he should have ICCM form.
Now register on your own - from “Village Clinic - Banda household - Register new client:(a) Female over 10: Memory Banda- Gender = Female- DOB = 6 December 2001 (age = 16)- Outcome: Family Planning form (if trained FP), CBMNC Registration (if trained CBMNC)(b) Female over 10: Tereza Banda- Gender = Female- Age = 11 years- Outcome: CBMNC Registration (if trained CBMNC) - no FP because too young(c) Male over 10: George Banda- Gender = Male- DOB = 10 May 1980- Outcome: Family Planning form (if trained FP)(d) Child between 6 and 10 - Age = 8 years- Outcome: Next day will be removed from the phone automatically because too old ICCM too young FP/CBMNC(e) Baby under 6 weeks- Age = 2 weeks (or DOB = today)- Outcome: Baby still in postnatal stage, will not be created. Go to mother’s profile

Change Client Details

- Go to Village Clinic - Banda Household - Find the client - Change client information- Change name - Matthew Banda to Matthew Bondwe- Change DOB - Tereza Banda new DOB is 30 March 1990

Remove Client

(a) Go to Village Clinic - Banda household - George Banda - Remove Client-Reason: Death- George should be removed from the phone(b) Go to Village Clinic - Banda household - Memory Banda - Remove Client- Reason: Relocation, to another district- Memory should be removed from the phone

Change Household details

- Go to Household Information - Banda household - Change household information- Address - Banda household, new address is “2 blocks past baobab”- Name - new name is: Bandwa household- After submitting the form, the details should change on the household

ICCM Cases

ICCM Scenario 1:

Register: Maruwo Household, then register new client = Peter Maruwo, DOB = 30/08/2015 (adjust accordingly, less than 5 years old)
(a) Sick no danger signs:- Open the ICCM form for Peter Maruwo- Cough = Yes, Days = 4- Red eyes = yes, Days = 2- Breath counter - over 50 breaths (tap 50 times)- No to everything else- Give treatment
(b) Follow up - better:- Open the ICCM follow up- How is the child now? Better- Referral should close
(c) Sick no danger signs + malaria:- Open the ICCM form for Peter Maruwo- Diarrhea = Yes, Days = 3- Fever = Yes, Days = 2- No to everything else- MRDT - “Submit now and fill in outcome later”- Note how you can start another ICCM visit while you wait for MRDT results- Navigate back to the child and fill in MRDT Details- MRDT = positive - Give treatment
(d) Follow up - danger sign- How is the child now? Has a danger sign- Refer- Follow up stays open
(e) Follow up again - 'better'

ICCM Scenario 2:

Register Ambwene Simukonda in the Maruwo household
(a) Danger signs:- Open ICCM form for Ambwene- Diarrhea = Yes, days = 14, blood in stool = yes- MUAC = Yellow- Treat and refer
(b) Follow up: Better- Did the child go to the health facility? Yes- How is the child now? Better- Referral closed

ICCM Scenario 3:

If you finish ICCM exercises, make up your own practice cases with more children, and different symptoms. Practice makes perfect!

CBMNC Cases

CBMNC Scenario 1:

Mother has twins, one passes away during postnatal period.
(a) Register new household: Kudebe.
(b) Register new client: Mary Kudebe, female, DOB = 30/10/1989
(c) CBMNC Registration: Pregnant- Open CBMNC Registration for Mary- Pregnant or postpartum? Pregnant- Gravidity = 2, parity = 2- Information known = LMP- EDD = next week Monday- Submit form - Outcome: ANC visits and Delivery Outcome available
(d) ANC:- Open the ANC visit for Mary- Is Mary still pregnant? Yes- Date of visit (today)- Pregnancy details correct? Yes- Tested for HIV? Yes- Danger signs - No to everything, yes to Severe Headaches- Name of referral facility: Nchalo, date of referral = today- Schedule next visit
(e) CBMNC Follow Up:- Because referred for danger sign, must do follow up, open CBMNC Follow Up- Previous referral info displays- Did she go to Health Facility? Yes- How is she now? Better- Follow Up closes, ANC available again
(f) Delivery Outcome: Twins- Open the Delivery Outcome for Mary- Is Mary alive? Yes- Delivery date (4 days ago), discharge date (yesterday)- What is the outcome? Delivery- Where did she give birth (choose any, make up the answers)- How many babies were born alive? 2- Status of Baby 1 now? Alive, Name: Elizabeth, Female, birth weight = 2800- Status of Baby 2 now? Alive, Name: Bright, Male, birth weight = 2200- Submit form. Go back to the Household - you can see 2 new babies in the household
(g) PNC Maternal- Open the PNC visit for Mary- Answer questions, no danger signs- Are you exclusively breastfeeding? No. Why? Choose any- Counseling on breastfeeding- Schedule next PNC visit
(h) PNC Neonatal (Refer danger signs)- Open the PNC Neonatal visit for Elizabeth Kudebe- Is Elizabeth alive? Yes- Danger signs: Signs of local infection- Current weight = 2750- Breaths per minute = 60- Temperature = 36- Enter referral details
(i) PNC Neonatal (Baby deceased):- Open the PNC Neonatal visit for Bright Kudebe- Is Bright still alive? No- Date of death- Where did Bright’s death occur? At home- Cause of death: Diarrhea- Bright’s case will now be closed
(j) CBMNC Follow up (PNC danger signs):- Open the CBMNC follow up visit for Elizabeth Kudebe- Previous referral info displays- Did she go to Health Facility? Yes- How is she now? Better- Follow Up closes, PNC available again

CBMNC Scenario 2:

Mother dies during Postnatal period.(a) New household: Your family name(b) New woman: Tamanda, age 22(c) Register Tamanda as pregnant in CBMNC Registration(d)Do the ANC visits:- One visit with danger signs- Complete CBMNC follow up, she is better- Another visit without danger signs(e) Delivery outcome: Mother (Tamanda) dies, but she had two babies:- One stillborn [recorded but not registered as a client]- One surviving baby: Blessings (Female, weight = 2600) [this baby is registered](f) Do the PNC neonatal visit for Blessings

CBMNC Scenario 3:

Mother has a miscarriage.(a) In Kudebe household, register another woman: Patricia, age 18(b) Register Patricia as pregnant in CBMNC Registration(c) Do an ANC visits - no danger signs- Sadly, Patricia had a miscarriage after ANC 1(d) Fill in Delivery Outcome: for miscarriage [no babies are registered automatically](e) Complete one PNC Maternal visit for Patricia to check how she is doing

CBMNC Scenario 4:

Make up the details on your own. For example, healthy mom, with one baby or multiple children.

Family Planning (FP) Cases

If the details are not provided in the exercise, enter any details (eg. registration numbers, parity etc)

FP Scenario 1:

Jean Bondwe on Family Planning, Village Clinic method (Ovrette)
Register Household: Bondwe. Register woman: Jean, 20 years oldOpen Family Planning form:- Are you using a family planning method right now? = Yes- Which method are you using? = Ovrette / progestrin only pill- Are you still using method? = Yes- Are you satisfied with method? = Yes- Do you want to continue using this family planning method? = Yes- Follow up on pills shown- Give pills: 3 cycles- Risk assessment: Have you been tested for HIV before? No. Do you want to be tested? Yes- Referral for testing- Schedule next visit for 3 months time

FP Scenario 2:

Mary Tamani new to Family Planning, Chooses Village Clinic method (Depo)
(a) Register household: Tamani. Register client: Mary, 18 years old(b) Open Family Planning form:- Are you using a family planning method right now? = No- Have you ever used a Family Planning method before? = No- Either show counselling on the phone or use Kulera Flipchart- Pregnancy screening - Are you pregnant? No. Did you have unprotected sex? No (passes screening)- In medical screening - say No to everything (so that she passes medical screening)- Choose method: Depo Provera IM- "You have chosen a method which I can give to start using today."- Give injection- It will say her due date for next depo is in the past

FP Scenario 3:

Mary Tamani Late for Depo(a) Find Mary Tamani in the Village Clinic(b) Open Family Planning form:- History section - update her FP registration number “Update general history”- Date of last depo shows, Is this correct? No (can always verify what the date was, even though phone remembers from last time)- Put the date as 4 months ago (to make her late today)- Mary Tamani is late for her depo injection, it was due on (Date)- Goes to Late Screening questions: Sex since depo = yes, condom = no, period = yes- Because she passes screening, allowed to get depo

FP Scenario 4:

Tasokwa Nchere unsatisfied with VC method, chooses new VC method (a) Register household Nchere and woman: Tasokwa age 21(b) Open Family Planning form:- Are you using a family planning method right now? = Yes- Which method are you using? = Male Condoms- Are you still using method? = yes- Are you satisfied with method? = No- Do you want to continue using this family planning method? = No- Pregnancy screening - Are you pregnant? No. Did you have unprotected sex? No (passes screening)- In medical screening - say No to everything (so that she passes medical screening)- Choose method: Combined Oral Contraceptives

FP Scenario 5:

Memory Manduwa, on Family Planning, HC method and unsatisfied (IUCD)(a) Register household Manduwa, and woman: Memory Manduwa age 30Open Family Planning form:- Are you using a family planning method right now? = Yes- Which method are you using? = IUCD- Are you still using method? = yes- Are you satisfied with method? = No- Do you want to continue using this family planning method? = No- Outcome: Should refer them to Health Centre because unsatisfied
Follow up is created, open Follow Up:Did you go to the health facility to try to complete this referral? = YesDid you receive counselling about the method? = YesWhat was the outcome of your counselling? I am starting a new methodHas your previous method already been removed? YesHave you already started a new method? Or know which one? = No I need help choosing“Carry on with the family planning form to help client choose a different method”
Open Family Planning form again, should start straight at the "choose new method" sectionChoose method: Combined oral contraceptives

FP Scenario 6:

Grace Mzungu, mew to Family Planning, Choose HC method (Jadelle) then follow up (got IUCD instead)
Register household - Mzungu, Register Client - Grace, age 21Open Family Planning form:- Are you using a family planning method right now? = No- Have you ever used a FP method before? = No- Pregnancy screening - Are you pregnant? No. Did you have unprotected sex? No- Choose method: JadelleShould refer them to Health Centre because they want a health centre methodFollow up is created, open Follow Up:- Did you get Jadelle? = No- Did you get a different method from HC? = Yes- Which method? IUCD- New method saved for next visit, no need for another Family Planning visit unless they need anything else.

FP Scenario 7:

Jean Bondwe stopping all methods to fall pregnant (Village Clinic Method)
Find Jean Bondwe in the Village ClinicOpen Family Planning form:- Are you still using method? = Yes- Are you satisfied with method? = Yes- Do you want to continue using this family planning method? = No- Why are you discontinuing this method? (want to fall pregnant)- Because Jean wants to stop to fall pregnant, explain how long it takes for fertility to returnLater when she has finished giving birth etc, she comes back for Family Planning, open visit form again:Last visit on record Jean stopped all family planning methodsStarts asking original visit questions (on any method, etc.)

Stock Management

Stock Scenario 1

Capture stock received: add 20 of each commodity into your app.
This is so that we have a positive stock level.
Go to View Stock Levels and they should all be 20.

Stock Scenario 2

You received the following drugs from Lodjwa Health Centre. Capture the stock received (stock item, amount):- LA 1 x 6: 60 packs- LA 2 x 6: 60 packs- Paracetamol: 1000 tablets- ORS: 100 sachets
View your new levels.and see whether they are entered correctly.
Your colleague HSA Lumbani ran out and so you gave him 30 LA6x1 and 20 ORS. Capture the adjustments with the correct reason.
By the end of the month you realised some drugs have expired. Capture the losses, by deducting from your total stock level and giving the correct reasons (list of expired items, amount expired):- Paracetamol: 100 tablets- Condoms (male): 25- ORS: 20 tablets
View your new levels.and see whether they are correctly adjusted.
