Workshop Checklist

1. Select topics from the list here and schedule an in-person workshop.

2. Submit workshop schedule to Digitiunity no later than 3 weeks prior to each workshop. 

3. Keep a record of participation data using a sign-in sheet or something similar.

4. Use this media release if you plan to take photos (please send them to connectedlearning@digitunity.org).

5. AT&T requires we collect participant and facilitator feedback using the surveys linked below.

6. Submit this workshop reporting form within 3 business days of any completed workshop

7. Submit this MONTHLY workshop reporting form no later than the 5th of each month for the previous month. (If no workshops were held in the previous month, a reporting form is still required)

* The participant survey does not require any identifying information, but will include an option to provide a testimonial, with first name and last initial required. All participating organizations must to submit a minimum of 4 testimonials with photos prior to the end of the program as a requirement to receive stipend funds . AT&T uses survey data to improve program materials and reports aggregate data in its Environmental, Social, and Governance reporting.
