• Birth Certificate

• Nurse Form

• Auto Pay Form (if you choose to opt in)

• Registration Fee ($110 to register)

• Immunization Record

All forms, registration payments must be turned in to complete your child's enrollment in our Early Childhood Programs. Please note that class offerings do need a minimum number or registered students for them to run. If the class does not meet the minimum, you will be notified.
