School-Located Influenza Clinic Fall 2024 (students only)

The flu vaccine will be offered at your child's school through the School-Located Influenza Program this

fall. This program makes it easy and convenient to keep your child, the school, and the community

healthy throughout the year.

The flu shot and nasal spray (FluMist®) will be offered as available.

Your school’s flu clinic will be held on Thursday, September 19th in the morning. Registration is required and will only be available from

August 7–September 11 at 11:00 p.m. No late registrations will be accepted.

Important information to know:

 Registration is required to receive the flu vaccine.

 Registration forms will not be accepted after September 11.

 Non-medication pain-reducing options will be available.

 Health insurance will be billed. If you don't have health insurance, your child can still get the flu

vaccine at little or no cost.

 Provide a reminder to your child the day before receiving the flu vaccine and have them wear

clothing that allows access to the upper arm.

Online registration is preferred. Paper registration forms will also be available on meet the teacher conference days. We

strongly encourage you to take advantage of this important opportunity.  Questions can be directed to

Dover Eyota Middle school/High school will host a community flu & covid vaccine clinic October 15th 3:30-5:30pm in the cafeteria. Any ages welcome!

No pre-registration required, just show up and bring your ID and insurance card.  Acceptable insurance will be taken.  Participants without acceptable insurance or uninsured can pay by cash/check.  Flu shot $40, FluMist (nasal spray) $45, Senior Dose (ages 65+) $70, covid $185
