Requests and supporting documents shall be submitted through an online form and then processed by the personnel-in-charge. Upon releasing, the client will receive the requested document electronically or via courier convenient to the client. Submission of physical documents may be required, if necessary.
Requirements shall be submitted online for checking and verification. Once found sufficient, physical documents shall be submitted via any courier available or requestees may schedule an appointment at SDO. Otherwise, all discrepancies must be corrected first.
Processing of requests shall only begin once these documents have been handed in.
Steps in Processing Online Requests
Access Online Services Portal Site.
Click the ONLINE REQUESTS link and choose your request.
Prepare Requirements for your request.
Fillup Form and upload digital copy of the requirements..
Click SUBMIT.
Releasing of Requested Document
Submit four (4) Certified True Copies of the physical documents you uploaded via LBC or any courier convenient to you.
You will receive an email or text that your document is ready.
Book a Grab Express Delivery to pickup your document

After receiving an email or text from us, you may schedule a visit to SDOQC and bring the Certified True Copies of your physical documents to pickup your document.
