You can request the following documents through this form:

• Certificate of Good Moral Character
  A statement confirming the individual’s conduct and behavior during their time at the school, often required for job applications or further education.

• Certificate of Enrollment (Bonafide)
  An official certificate verifying that the individual is currently enrolled in the school, typically used for scholarships or other official purposes.

• SF9 (School Form 9/Form 138)
  A report card containing the student's academic grades and performance, usually required for transferring schools or further education.

• SF10 (School Form 10/Form 137)
  A cumulative record of a student’s academic history, often used for admissions or transferring to another educational institution.

• Diploma
