Request for Certificate of Enrollment
1. Parent or pupil will inform the class adviser about the specific request.
2. The class adviser will then verify attendance of the pupil and fill-out the necessary data in the online request form.
3. The School Records Custodian will prepare the Certificate of Enrollment to be signed by the School Principal.
4. Once signed, the class adviser will be notified to get the signed Certificate of Enrollment from the Principal's Office.
5. The class adviser will then release the Certificate of Enrollment to the parent or pupil.
NOTES:
-Releasing of Certificate of Enrollment is every Monday and Thursday only.
-Parents do not need to go directly to the Principal's Office to request for Certificate of Enrollment.
-Only 1 request per school year is allowed, except for VALID/LEGAL reasons.
