The ICT office only requires you to accomplish the Internet Connection Plan-Usage Agreement (ICPUA) form which can be downloaded at this link: http://tinyurl.com/ICPUAForm.

NOTE: The ICT Unit will NOT need a printed copy.)


Once received, the Division IT Officer shall review the information and sign it. The signed ICPUA shall be sent to the School DepEd Microsoft 365 account (or the School Head's DepEd Microsoft 365 account). Download and submit it to the Division Accounting Office together with the Internet Connection Request Letter (ICRL). You may download the Internet Connection Request Letter template in this link: http://tinyurl.com/ICR-Letter


The Internet Connection Request Letter (ICRL) is addressed to the Office of the Schools Division Superintendent and attention it to the Accounting Office. The recommending approval will be by the Accounting Office and the approval of the request  will be by the Office of the Assistant Schools Division Superintendent of your school’s district/cluster.
