Be REMINDED of the following, before filling-out a request form:

1. For Newly Created Google Workspace account owners, NO NEED TO REQUEST for a DepEd Microsoft 365 account because we will automatically send a request to the DepEd Central Office - ICTS by forwarding your Google Workspace details to them. If after a month, you still haven't received your DepEd Microsoft account, request for a follow-up. 

2. Pursuant to Republic Act No. 10173 (Data Privacy Act of 2012), we do not process requests for an individual made by other people. That individual must personally submit the request using their own account.

3. ONLY THREE (3) password resets are allowed within a year. When the requestor exceeds the number of allowable password reset, he/she should write a letter addressed to the Schools Division Superintendent attention to the It Officer stating the reason why the account is to be reset again. Invalid reasons may lead to account suspension.

4. Provide all the necessary information with utmost accuracy.

5. Limit your request to one message only. Flooding or multiple submissions for one or same request within a day will result in its cancellation.

6. By submitting the request, you agree to the terms and conditions of Microsoft and activate the account 90 days after receiving your O365 account/School O365 account/O365 School Admin account credentials.
