Instructions for LAC Facilitators

1. Based on the individual development, eSAT, Mid-Year Review, and other observable needs of the teachers and staff, complete the LAC Matrix for the first half and second half of the school year, using the template for LAC Facilitators.

2. Make a copy of this template for LAC Facilitators and rename using this format:

SURNAME, FIRST NAME LAC PLAN AND REPORT FOR LAC GROUP SY

Example: ARMAS, ANGELO LAC PLAN AND REPORT FOR SCIENCE DEPARTMENT 2022-2023

ARMAS, ANGELO LAC PLAN AND REPORT FOR HEAD TEACHERS 2022-2023

3. Accomplish initially the template by completing the data in each tab the Info, Members, and Matrix.smea

4. Send the LAC Matrix to the LAC Leader (Schoolhead) for approval.
