DEER PARK HIGH SCHOOL ATHLETIC HALL OF FAME

Kindly read over the criteria that must be adhered to in order to be eligible. If you would like to nominate a candidate please click on the appropriate link to complete the nomination form. If you submitted an application in 2022-25 that application is still valid for the next four years. All nominations will be kept on file for continued review for a period of up to five years.

CRITERIA

Athlete

2-time All-County in one sport, or All-County in 2 sports, or All Long Island / All-State in one sport.
Minimum of 5 years out of High School.
Must have graduated from Deer Park High School.
Reputable character/leadership qualities.

Athlete Nominee Form

Team

County Champion or better.
Minimum of 5 years since accomplishment.

Team Nominee Form

Coach

Served as a Deer Park Coach for at least 10 years.
Outstanding contributions to Deer Park Athletics.
Significant accomplishments during Deer Park tenure.
Leadership above and beyond everyday participation.
Retired from coaching for at least 3 years.

Honorary

Does not qualify under the other categories.
Has made significant contributions to Deer Park Athletics in our district.
