Sport Registration and Clearance Procedure
All student-athletes and their parents or guardian must make sure that all forms are signed off by the parent and student on FINAL FORMS and that the student-athlete has an up to date physical to participate in sports. If these forms are not complete the student-athlete will NOT be able to tryout. NO EXCEPTIONS!
Final Forms allows you to complete and sign required athletic participation forms from your computer, tablet or mobile device. Registration is open 30 days prior to the start of each season.

NYS PHYSICAL FORM- REQUIRED TO BE FILLED OUT YEARLY
Must be uploaded to Final Forms to be eligible to attend tryouts.
