PRE-TRIP MEETING INFORMATION

Below is the information shared at the Trip Parent's Meeting held on Wednesday, February 21st at 6:30 pm.  

All families are responsible for reviewing this information.  Click on links or find files below:

• Overnight Medical Form - THIS MUST BE SIGNED and RETURNED BY EVERYONE NO LATER THAN MARCH 1st.

  • Everyone must sign Page 1 & 3

  • Complete and sign Page 2 IF your student is taking any medications with them (either prescription, OTC, or supplement)

  • Complete and sign Page 4 if you would like us to be able to administer any of the listed medicine during the trip.

• Administering Medication Form - RETURN NO LATER THAN MARCH 1st

  • Complete and return IF your child is taking any type of medication during this trip.

  • Complete one form for each medicine you child is taking on the trip.

  • You need a Physician's Signature for

    • Prescription medication

    • Non-FDA Approved Supplements (various vitamins, supplements, melatonin, etc)

    • Any over-the-counter medicine that it taken in doses above the instructions on the label.

  • You do NOT need a Physician's Signature for:

    • Over-the-counter medications that you are taking as directed.
