2022 DCHS Graduates - FINAL Transcript Request Form

This form is ONLY for 2022 DCHS Graduates!
ALL FORMER DCHS graduates must order FINAL Transcripts from the Douglas County School System. Go to dcssga.org, click Departments, then Transcripts. You will need to order online through them.
*** This form should ONLY be filled out by CURRENT DCHS students. ***
Steps to order your FINAL Transcript to be sent:
Pay for your FINAL Transcript. Make sure to keep track of your receipt number. You will need this for Step 2. You can go to dchs.dcssga.org, select Resources, then Student Fees Online Payment, then select "Transcript". Here is the direct link to pay. https://douglascountyhs.revtrak.net/
Fill out the Google Forms below.
*** This is the form when ordering a FINAL Transcript at the end of the school year ONLY. The earliest these are sent out is mid-June. This is the same timeline as every school year and all colleges are aware of this time frame. ***
