State Retirement Plan
Long-Term Disability Coverage
Is through NPERS - Nebraska Public Employees Retirement System
Is mandatory of all administration staff that are 1/2 to full-time. (20 hours per week minimum status to be eligible) and eligible upon hire or rehire
Employee contribution rate is 9.78% and Employer contribution rate is 9.88%. (Set by the State of NE).
An employee is fully vested after five years in the plan. Vesting time requirement may take longer to meet if less than full time.
Available for 12 month staff only
Is through Madison National Life Insurance (parent company is National Insurance Services)
Premiums are .44% of your salary. 100% paid by the employee over 12 months. For Superintendents, health insurance is included in with the salary to determine premiums.
If not taken upon hire, an Evidence of Insurability (EOI) form is required to be completed.
403B for Extra Retirement Savings
Vision Insurance
Extra type of Retirement Savings Plan that is optional and voluntary. Eligibility falls under the same guidelines as the State Retirement Plan (20 hours per week minimum status to be eligible)
403B accounts are administered by a third party - Union Bank
Employee gets to choose the dollar amount for payroll deduction amount and also gets to elect their investment fund accounts.
Maximum contribution limits are governed and set by the IRS
Is through VSP - Vision Service Providers
Provides coverage for services including eye exams, eyeglasses, contact lenses and some Lasik surgery. Copays may apply to some services.
This benefits is paid 100% by the employee.
Premium cost is paid over 10 months for school year staff and 12 months for full year staff.
There are four policy levels available: Employee Only, Employee/Spouse, Employee & Child(ren) and Family.

FSA Accounts are administered by Payflex
This is an election that is governed by the maximum contribution limits and Section 125 laws by the IRS.
Two types of accounts to choose from:
Health Care - for out of pocket medical, dental, prescription and vision expenses
Dependent Daycare - for our of pocket expense for daycare up until age 13 order older for a qualifying handicapped child. Must be a licensed daycare provider with the state.
Unused balance do not roll over if not used up. "Use it or Lose It"
Applies only if employee carries or elects a high deductible health savings account eligible health insurance plan.
If employee carries a high deductible HSA eligible health insurance plan outside of DCPS, then proof in writing from their insurance company will be required in order to participate in HSA payroll deduction election.
The pre-tax benefit can be taken as a voluntary payroll deduction amount.
Maximum contribution limits are set by the IRS.
District contributes only to staff who works full-time (12 months).

Comes as a package of health and dental together OR can be dental only OR all waived. Cannot be health only election.
District pays for employee only premiums for school year staff. Staff who work full-time, 12 months, the district pays the family premium. Staff who work during the school year are able to pay the difference if spouse/child(ren)/family coverage is selected.

EHA Wellness is a voluntary health improvement program brought to you by the Educators Health Alliance (EHA). It is offered to all employees of EHA member groups and supports affordable health insurance. Participants can choose to participate in any or all of the opportunities offered throughout the year.
