Sports Physicals

Students in Athletic Activities must have a current sports physical on file in the Activities Office to participate (excluding Unified Basketball and Adapted Bowling). 

Sports Physicals expire three years from the date of the exam.

Well-child checks do NOT qualify as a sports physical.

Once the physical form is completed by your doctor, please email a copy to Taylor Roles, DC Activities Secretary, at taylor.roles@dc.k12.mn.us and keep the original for your records.
