The Football Legends with support from ActiveSG Football Academy will conduct a FREE football clinic for DAS students from Preschool to Secondary 2.
If your child is interested in learning some football tips from the legends or just want to be in the arena with them you need to sign up quickly!

- I will bring my child to the Football Clinic at 4:00pm to register for the clinic.
- I will ensure that my child is wearing the correct football attire including appropriate football boots, shin pads and football socks.
- I understand that I can watch my child during the clinic from the stands, however, when the clinic is finished I must vacate the stands.
- I understand that I will be at Jalan Besar Stadium to collect my child at 6:30pm at the Tyrwhitt Rd, Stadium Entrance.
- I understand that if I wish to stay for the events following the clinic I will need to purchase a $20 ticket per person.

- I hereby agree for my child to participate in the DAS Football Legends Clinic organised by DAS and ActiveSG Football Academy at Jalan Besar Football Stadium on Sunday 6 October 2019 .
- There is a potential risk of injury in this activity. My child does not have any medical ailments, physical limitations, or mental disabilities that could affect his/her ability to participate in this activity. My childs involvement and/or participation in this activity is voluntary and at my own risk.
- DAS does not undertake any direct or legal responsibility for my childs personal safety or well-being when participating in this activity. I hereby release DAS and each of their affiliates, as well as their respective officers, directors and partners from any and all claims and causes of action that result in my childs participation in this activity.
- I agree and consent to DAS photographing and videoing this activity which my child is participating in and can use the images, data and video footage for future promotion and marketing of DAS and its activities.
- I have understood these conditions and I agree to allow my child to participate in this activity.
