This form is to be accomplished by the Local Chief Executive of the nominee.

LGEMC Form B: LGU Screening Certification

This form is to be accomplished the HR or Admin Officer.

DECLARATION OF MEDICAL ILLNESSESS

LGEMC Form D:
Government Physician's Certification
