The following forms should be accomplished and submitted by the nominee
(Department Heads/Officers In-Charge of Critical Local Government Functions).

By accomplishing the following forms, you agree with the storage and handling of your data by the DAP in accordance with the Data Privacy Act of 2012.

The nominee must also submit additional requirements listed below:

Updated Personal Data Sheet – CSC Form 212
Scanned Copy of Appointment Papers (CSC Form 33)
Scanned Copy of Designation Order (if applicable)
Copy of Organizational Chart

LETTER OF CONFORME

Step 2.1: download a copy of the Letter of conforme (LOC).

you will be required to submit the signed document once ACCEPTED into the program.

Note: A processing fee will be collected should the scholar withdraw prior to the start of and during the program.

Step 3: Once initial screening is done, successful applicants will be notified to undertake online assessments and interview to complete the application process.

Step 4: submit hard copies/original documents to dap office.
