Any medication (other than Tylenol), that a student is taking in school needs a medical authorization form filled out by the provider. Unless the student is deemed self carry by the provider, a parent must deliver the medication to school. Parents may sign a Tylenol permission slip to allow the nurse to administer tylenol. 

Current orders and non-expired medication(s) are needed before a student can participate in a field trip.
