Transcript Request Instructions
COMPLETE YELLOW FORM. Your transcript will not be sent unless you fill out your form.
Your yellow form is kept in the Counseling Office.
You will need to know the following information when filling out your form. Please do your research!
Deadline- ROL, REG, EA, ED?
How are you applying?
Application type?
Letters of Recommendation- are they required or optional? How many letters do you need/allowed to send?
*SAT/ACT Scores are on your transcript. You MUST ask for them to be removed* See checkbox at the top of the form.
Common Application Information
Please note if you’re using The Common Application - do not request letters via Common App. Please request letters by talking with your teachers and submitting requests on Naviance.
Students using The Common Application when applying to colleges will need to go through the set-up process to link The Common Application and Naviance. Click Match ACcounts in your Naviance Account. It will redirect you to your Common Application Portal. You will need to make sure you completed the FERPA in your Common Application as well!
One and Done Process -After 1 submission of your initial transcript, school report, school profile, counselor recommendation, fee waiver, & midyear report, Common App will automatically deliver the documents to any additional colleges that you add to your Common App list
Letters of Recommendation Information 📝
Check the university’s admissions webpage for admission requirements to determine if letters of recommendation are required, optional, or not needed.
Complete the LETTER OF RECOMMENDATION FORM in Naviance. This form will help create quality letters of recommendation. You must talk with teachers first and then request the letters in Naviance. Counselors are not listed in Naviance. You must talk with your counselor before noting that a counselor letter is required on this form.
Instructions for how to request a letter of recommendation in Naviance :
Click on Colleges at the top of your home screen
Under the Applying to College section, click on Letters of Recommendation
Click Add Request
Select the teacher
Select General Request
Submit Request
Repeat if needed
