Cautions

• Only one member from each team should fill up this form. Multiple submission are strictly not encouraged.

• It is not guaranteed that your first choice of supervisor will be assigned. Project/Internship Committee will assign a supervisor according to the rules and regulations advised by the honorable head of the department.

• After assigning a supervisor, you cannot change your supervisor before Title Defense. Changing supervisor is not appreciated at all.
