Hester Athletic Forms

Hester parent/guardians along with their student(s) need to fill out five forms in order to participate in Hester athletics. All forms are described and accessible on this page.

Sports Physical Form

In order to participate in athletics, students need a sports physical every year. All incoming 6th graders are required to have a physical upon entering Hester Junior High School so they are covered for their 6th grade year. Physicals are good for one year, so students will need to have a sports physical completed each year they plan on participating in Hester athletics.
