Departing DMS at approximately 9:15 am

Returning to DMS at approximately 7:00 pm - 7:30 pm

(Buses/vans will be provided to students and adults for this field trip)

EVENT LOCATION:

Six Flags Great America

Gurnee, IL

DESCRIPTION OF ACTIVITY:

All eligible 8th-grade students will spend the day at Great America to celebrate the completion of three years of middle school.

COST:

$22.88 - Food (all students will pay this)

$43.68 - Ticket (students with a Great America pass will not need to pay this)**

**Student must bring and show their Great America season pass on the trip day.
(Failure to do so will result in a charge for the ticket price)

OTHER IMPORTANT INFORMATION:

Food cost includes a Mega Meal Deal Voucher*

(*one meal, one snack, and a free unlimited drink upgrade)

Students may NOT bring in outside food or drinks.

Parents will need to be at DMS at 7:00/7:30 pm for pickup on that day.

Students are expected to follow all expectations set forth by both the DMS administration and Great America's Park Rules.

If expectations are not met during the trip, parents should be prepared to pick up their child immediately from Great America.

EMERGENCY MEDICAL AUTHORIZATION & MEDICAL CONSENT FOR TREATMENT

Permission is hereby granted to the attending physician to proceed with any medical care which may become necessary in the case of injury, accident or a medical emergency involving the above named student. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that an attempt will be made by the attending physician to contact me in the most expeditious way possible. If said physician is not able to communicate with me, the treatment necessary for the best interest of the above named student may be given.

ASSUMPTION OF RISK, WAIVER OF LIABILITY AND HOLD HARMLESS LIABILITY RELEASE

Parent/legal guardian agrees to hold harmless and to indemnify the School District from all claims, actions, damages, and liability in connection with any act by your student that would directly lead to the loss of life, personal injury, and/or property damage.

In the event this field trip is postponed or rescheduled for a later date, this permission slip is valid for the new date. 

 PLEASE NOTE: Payments using a debit/credit card will incur a 2.9% processing fee charged by the card processor. The total fee is displayed prior to submitting your final payment. However, if you prefer to pay via eCheck, there are no additional fees incurred.
