Community Consolidated School District 168 Health Services
Each school health office is staffed by a Registered Nurse. The nurses provide emergency care to students who are injured at school or during school-related activities. The nurses also collaborate with parents to address the needs of chronically ill children so that the children can remain in school. Health office procedures and requirements are regulated by Illinois School Code and the Illinois Department of Public Health.
Physical, dental and eye examinations and immunizations, prescribed by State Law, are required of students in the categories below. Parents are requested to have their children examined by a licensed practitioner and to return physical, dental and eye examination forms and immunizations records to school before the school year begins. All physical exams and immunizations must be filed by October 15 of the school year, or by district policy, the student will be excluded from school.

If not, your child will NOT be allowed to attend school until the exam has been received.
NEW PRESCHOOLERS
Physical Examination - Must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history signed by the parent. This must be submitted to the nurse within 30 days of enrollment.
KINDERGARTEN
Physical Examination – Must be completed within one calendar year prior to the date of entry. The exam must include up-to- date immunizations and medical history signed by the parent.
Eye Examination - Must be performed by a licensed optometrist within one year prior to the first day of school and submitted before October 15 of the school year.
Dental Examination - Must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
2ND GRADE
Dental Examination - Must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.
6TH GRADE
Physical Examination - Must be completed within one calendar year prior to the date of entry. The exam must include up-to-date immunizations and medical history signed by the parent. Sports Physicals do NOT count as a school physical.
Proof of one dose of the Tdap vaccine for students entering sixth grade.
Proof of one dose of MCV4 vaccine on or after the 11th birthday.

NEW STUDENTS FROM OUT OF STATE

STUDENTS TRANSFERRING FROM OTHER ILLINOIS SCHOOLS
Transfer of current Illinois school health records (This includes: physical exam with up-to-date immunizations, dental exam and eye exam). Must be submitted to the nurse within 30 days of enrollment.
