Step 1: Get a report from the optometrist or ophthalmologist that provides a diagnosis and corrected acuity. The doctor's office may have a form, or you may print out our form and take it to the provider.
Or:
Fill out a release of information form with your eye doctor's information and return it to the school nurse or case manager.
*Kaiser Permantente has a specific release form.
Step 2: The school nurse or case manager will contact a member of the Vision Team to discuss the results of the eye report. If the report indicates a need for further evaluation, the parent will be contacted to schedule a meeting at the school.
Step 3: Parents/Guardians will work with the school to develop an assessment plan to determine whether the student would benefit from vision services. Students cannot be assessed for services without permission from parents or guardians.
Step 4: An Individualized Education Plan, (IEP), team, including parents or guardians, will meet do discuss the results of the report. If the student requires services and/or accommodations, an IEP will be developed at this time.
