Please TAKE YOUR TIME filling this form out if you are interested in having me write you a letter of recommendation. The form will close on August 31, 2026 so you have lots of time to thoughtfully reflect on the prompts below before responding. I will only be writing 10 letters this year, I will send out an email to you on September 4th confirming your letter of recommendation. If you do not hear from me on September 4th, it means I am not writing you a letter this year.

I am going to be honest about your effort and performance in my class. Do you still think I should write your letter for you? Or is there a teacher who may be able to provide a better recommendation based on your performance in their class.
Did you cheat or plagiarize in my class or in any of your other classes? If so, I will not write you a letter...I have access to your behavior record and that is one of the first places I will look before agreeing to write your letter.
In addition to your academic performance, I will be asked to rate you in various personality traits. Here are some common ones I will be asked...each school varies in what they ask, with the Ivy League schools usually asking the most: sense of humor, leadership, industry, ability to work with peers, respect for adults, quality of writing, intellectual promise, maturity, motivation, reaction to setbacks, concern for others, Initiative. You do not have to score highly on every single one of these, but ask yourself- do you think you demonstrated these in my classroom? If not, you may want to find another teacher to write you a letter of rec.
Did you care about learning? Or points? I know the answer to this question, and I will be honest with you about it. During Trojan Time, did you approach conversations in a "what can I learn from this?" or "why didn't you give me these points"?
Did you participate? If not, you should find another teacher who's class you may have participated more in.
Were you prepared? Did you try your best? You did not need to earn an A in my class to earn a letter of rec. In fact, colleges much prefer to see letters for the C or B student who worked hard over the student who showed up, didn't participate, and got an A.
During independent work time, were you on task? Or were you constantly working on your other classes work?
Were you consistently on your phone in class? If your phone was a problem in class...you may want to ask another teacher.
How did you interact with your classmates?
How did you overcome difficulties?
How many assessment days did you miss? Were you frequently absent? When you were, did you communicate?
Did you take responsibility for things? "I didn't know...."...did you?
