Health Requirements
Pre-School
Health Physical completed by October 15 of the current school year
Complete Updated Immunization Record signed and stamped by a doctor
Tetanus/Diphtheria/Pertussis
3 doses of DTP or DTaP by 1 year of age. One additional booster dose by 2nd birthday
Polio
2 doses by 1 year of age. One additional dose by 2nd birthday.
3 doses for any child 24 months of age or older appropriately spaced
Measles
One dose on or after the 1st birthday
Rubella
One dose on or after the 1st birthday
Hib
4 doses
Hep B
3 doses appropriately spaced, 3rd dose must have been administered on or after 6 months of age
Pneumococcal
4 doses
Varicella
1 dose on or after 1st birthday
Kindergarten
Health Physical completed by October 15 of the current school year
Complete Updated Immunization Record signed and stamped by a doctor
Tetanus/Diphtheria/Pertussis
4 or more doses with the 4th dose received on or after the 4th birthday Tetanus/Diphtheria/Pertussis
Polio
4 doses with the last dose qualifying as a booster and received on or after 4th birthday
MMR (measles, mumps, rubella)
2 doses, first dose on or after 1st birthday, 2nd dose no less than 28 days later
Varicella vaccine
2 doses, first dose on or after 1st birthday, 2nd dose no less than 28 days later
Lead Screening found on second page of health physical form
Dental Examination dated between November, 2022 - May 15, 2024
Eye Examination dated between November, 2022 - May 15, 2024
Second Grade
Dental Examination dated between November, 2022 - May 15, 2024
Health Requirements
Ninth Grade
Health Physical completed by October 15 of the current school year
Dental Examination dated between November, 2022 - May 15, 2024
Twelfth Grade
Meningococcal Vaccine two doses of MCV4 vaccine (meningococcal conjugate vaccine). Only one dose is required if the first dose was received at 16 years of age or older.
If your child will be trying out for any sport, they must have a sports physical form on file prior to the season’s first practice. Incoming 6th and 9th graders may use a school physical in lieu of a sports physical, however a sports physical CAN NOT be accepted as proof of a physical examination for mandated physicals. Sports physicals are valid for 395 calendar days from the date the physical was issued.

3 doses of Hepatitis B, if not received previously
Meningococcal
