Risk Assessment Reference for Staff
When a student makes a statement indicating self-harm towards themself or others, a threat assessment must be completed prior to them leaving school that day. The assessment may also be completed the following school day if staff finds out about the statement after the student has left school. This assessment is completed by Ms. Shanna, Ms. Tricia, Ms. Lewis, or START Therapist. Please let us know immediately if you see any of the following:
Some examples of statements of self-harm:
I’m going to kill myself, I’m going to kill __________
I want to die
I don’t want to be alive anymore
The student may mimic self-harm (i.e. wrap a jump rope around their neck or make stabbing motions towards themself)
