Forms

As with almost everything in the modern world, some paperwork is required. On this page are links to some of what we require.

• Athletic Participation Form -- required for any activity with Cupertino FC (including tryouts and guest play)
• General Release -- parents are responsible for getting players to/from practices and games
• Emergency Authorization -- authorize potential medical treatment and assumption of risks

Also, we have a "player pledge" which can be useful to ensure that players and their families both understand what we expect of those involved in our club.
