Upon enrolment at CT Tutoring Center, a deposit of R1500 is payable upon submission of this form.
A scanned digital copy of this form will be made available to the parent/guardian for reference. If required, a photocopy may be provided upon request. CT Tutoring Center will securely retain the original signed document for its records.
• All payments to Impaq must reflect the student number as the payment reference.
• Once the application is submitted, Impaq will provide a quotation. Please note that:
• The quoted amount is approximate and may change slightly once processed.
• CT Tutoring Center cannot be held liable for any changes in these amounts.
• Impaq will only release the prescribed textbooks once this deposit has been received.
If you choose the debit order payment method, kindly ensure that:
• A confirmation of banking details (not older than 3 months) is sent with the CT forms.
• The deposit is paid in full before activating the debit order.
• Proof of payment must be sent to both CT Tutoring Center and Impaq.
• You accept the debit order mandate via your banking app.
• This enables your bank to transfer funds directly to Optimi Home (Pty) Ltd (t/a Impaq) on your selected date.
• Textbooks will only be dispatched once this process is complete.
• Grade 12 applications (with Term 1 report card) close on 31 March 2026.
• Grade 10 & 11 applications (with Term 1 & 2 report cards) close on 31 July 2026.
• The 2026 term dates will be communicated at a later stage.
