Student Forms

The following forms are required to be completed by the parent/guardian of students attending the MCVSD Career Center.

The above list of forms can be dowloaded and completed OR completed in PowerSchool. 

All returning students have been issued their PowerSchool credentials while new students will be issued their PowerSchool credentials

In order to complete the forms, login into PowerSchool. Select FORMS from the Navigation list on the left-hand side. All required forms will be displayed for you to revise or edit. Please be sure to click on SUBMIT at the bottom-right of each completed form to save your changes.

One form that is not available on PowerSchool at this time is the Confidential Student Medical History form. The form is located below. The form can be printed to be completed by hand. It is critical that you complete and return the form to the Career Center by September 17, 2021. Failure to do so will result in your child being excluded from attending the Career Center until the form is completed and returned.
