All activity requests must be submitted on our "ACTIVITY REQUEST" form well in advance of the thoroughly planned activity. Submit requests to the Director of Class Activities. Activity requests are approved on a first come, first served basis. Activity requests should be submitted no less than two weeks prior to the activity. Activity request forms can be found in the main office.
A closed activity refers to any activity that only allows certain people to attend. If your activity is a closed activity, please indicate who will be in attendance. If your activity is not a closed activity, then all CHS students should be able to attend.

If a room other than the advisor’s classroom will be used for an activity and the activity will be concluded before 4:00 P.M. on a school day, a building use form will not be needed when submitting an activity request.
If the sponsor’s room will be required after 4:00 or any other room in the building is required, please submit a Building Use Form. To speed the process along, activity requests and building use forms can be submitted at the same time.
AUDITORIUM USE
Arrangement for technical assistance, equipment needs, etc. should be submitted well in advance of the activity to the activity director. You need to fill out a building use form. The activity director will check with the drama director to make sure that the use of the auditorium will not interfere with a production or drama rehearsal.

If non-CHS students (guests) are allowed at the dance, then guest passes should be available for students to fill out at least 30 days in advance of ticket sales. Crestwood High School dances are for high school age students. Middle school students or those over the age of 20 are prohibited. Administration reserves the right to deny any guest application for any reason.
Guest Pass Application Procedures for Dances at Crestwood High School
Before the dance, you must:
Fill out this form COMPLETELY. Only one guest allowed per CHS student.
Photocopy the guest’s CURRENT SCHOOL ID (if they are in high school) OR their driver’s license if they have graduated. Staple the photocopied ID to the back of this application. Anyone over the age of 20 is not allowed at the dance.
Turn this completed form and photocopied ID in when you pre-purchase a ticket.
In order to gain admittance to the dance, one guest per CHS student must:
Be accompanied by the CHS student that sponsored the application.
Show a current PHOTO ID.
Have a pre-purchased ticket. NO tickets will be sold at the door and
only 700 tickets will be sold total. BUY YOUR TICKET EARLY!
Produce all of these at the door on the evening of the dance.
It is imperative that the chaperoning of events be given strong consideration. Large activities, such as dances, etc., must have at least one chaperone per 50 students. When at all possible, staff members should be used as chaperones.
If any individuals outside of the CSD faculty are to chaperone an event or work with students, an iChat form must be completed. Anyone who is going to be in the building, around our students at any event, field trip, or chaperones needs to fill out an iChat form. iChat forms need to be submitted to Lisa Musa (lmusa@csdmi.org). Please allow 24 to 48 hours for the iChat form to be processed. You can ask the activity director or Principal to see the list of individuals that already have an approved iChat form. IChat forms are good for 2 years.
