To be reimbursed, you:

1. Must be a PTA member 

   1. Fill out the MEMBERSHIP FORM

   2. Pay the $10 membership fee

2. Fill out this reimbursement FORM

3. Volunteer for one PTA event 

4. Come to one PTA meeting 

5. Upload a PDF of your receipts and send to KenwoodLeadersPTA@gmail.com
