A certificate of insurance shall be provided to the Creighton Elementary School District #14.
The certificate holder shall name (please make sure your insurance policy matches this exactly):
Creighton Elementary School District No. 14, their officers, Governing Board, Superintendent, employees, and representatives
Pursuant to A.R.S. Section 15-1105, RENTER NAME agrees to procure, at its expense, and maintain during the term hereof, a policy of general liability insurance, against claims for bodily injury, death, and property damage occurring in connection with its use of any portion of SCHOOL OR BUILDING NAME and/or the school’s contents, which insurance shall name Creighton Elementary School District No. 14, their officers, Governing Board, Superintendent, employees, and representatives as additional insured and be primary and non-contributing to any coverage maintained by on behalf of the district. Such insurance shall have
minimum limits of $1,000,000 per occurrence, and RENTER NAME’S right to use and occupy the facility is conditioned upon the prior delivery of the insurance certificate.
