Nomination Process

Families

Families may nominate their child for CGaT using the online nomination form below or by submitting a paper nomination form, available in the front office at every school. Completed paper forms should be returned to your school's front office. Once testing is scheduled, families will receive a permission-to-test form by email or through their child's teacher. Testing is offered three times each school year.

Teachers/Staff

Teachers may nominate students using the online nomination form below or by completing a paper nomination form, available in the front office. Return completed paper forms to your site's CGaT teacher's mailbox. Once eligibility is verified, permission-to-test forms will be distributed. Testing is offered three times each school year.
