Complete the ATRC Incident Report (* only if broken or needing repair).
Pack all the student’s equipment including chargers and/or accessories in a box.
Use the ATRC Return Label to label the box. One half of the form should be placed in the box and the other half on top of the box.
Complete page 2 of the label titled: Warehouse Services Request Form to arrange for the equipment to be picked up from your school.
Scan and email your Warehouse Services Request to warehouseservices@cps.edu

There are a few troubleshooting steps you can take to minimize your student’s time away from their device:
Replace batteries: please start by replacing all batteries in the communication device. We recommend using brand new batteries.
If you open the battery compartment and notice corrosion you can attempt to clean corrosion using a Q-tip and white vinegar.
Re-record voice output on the device*. Sometimes recordings are erased or written over. This is a quick-fix without the student having to send in their device.
If you replace the batteries in your student’s device and it continues to work intermittently please let us know! This intermittent behavior can be difficult for our technicians to see when they receive a device for repairs. This may result in the same device being returned to the student due to the fact that it is working well when tested in the warehouse. To ensure that the device is replaced when it is first received please send the device with brand new batteries. Please also check to ensure that the device is off during shipping. When our technicians receive a broken mid-tech communication device their first step is to test the batteries. If they note that the batteries are new then they will know to replace the device. In this case a replacement device will be sent to the student with your new batteries inside.
