Mail in Form

For your convenience we have developed the Mail in Donation form (found below) to be used if you wish to print and mail the form with your donation. This form may be used two ways.

1. Type into the form, print it and mail with your donation
2. Print the form and fill it out manually, send it with your donation.

Observe that these forms may request a little more information than usual. The reason is we offer many more options than most foundations

Options

1. Donation, One may contribute Cash, Stocks, Property,
2. Bequests to establish a fund or simply add to and existing fund.
3. We also offer the choices of direction your contribution to a specific area of interest such as Academics, Performing and Fine Arts, Athletics, General
4. One also has the choice of contributing on behalf of, in memory of, etc.

Each contribution will get a response letter indicating the receipt and the fact that it is a Tax deductible contribution to be used with your tax returns.
