MOVE UP Academy Ignite Online Learning Agreement

Student requirements: Due to the uniqueness of the online learning program, certain standards and behaviors are expected of students. Students are expected to complete the Online/Virtual Learning Readiness Assessment prior to requesting enrollment in the program. Please read through the following agreements and sign on the line provided.

As a student enrolled in online course I'm aware that:

I will need to create and maintain a study schedule that supports the timely completion of assignments, etc.
I will adhere to the school's timelines for completion of course requirements.
As a student, certain standards are expected of me. Appropriate use of the internet is expected at all times. All terms outlined in the district's acceptable use policy, student code of conduct, and academic honesty policy apply to this agreement.
All coursework and submissions that I do may be retrieved and or monitored by the teacher at any time.
I will follow procedures specific to the course or courses in which I am enrolled.
I must not inappropriately use information within the course.
I understand the attendance policy and procedure and commit to adhering to it.
I must follow all other rules as specified by the district, MOVE UP Academy, CHS, and online teacher.
Weekly contact with the teacher is mandatory. More than two unexcused weeks of lack of contact may result in removal from the program.
I will be committed to the course, adhere to the schedules as prescribed, and I understand that drops will be allowed only as outlined by the school policy and procedures.
I understand that the courses will be listed on my transcript or permanent academic record.
I must still take all local and state assessments as scheduled, if applicable.
I will use my first and last name when sending emails to CHS staff so there are no questions about my identity. I will also participate in online visual communication at least three times during the course.
I will not use the login or password of another student.
I understand that in person activities and/or assessments may be required to fulfill my course requirements.
I agree to have two way communication with my teacher on a weekly basis.
I agree to keep up with assignments/activities, quizzes, and tests.

Consequences for violations:

First offense: I will receive a verbal warning. My parents and administration will notified.
Second offense: I will receive a written warning. My parents and administration will be notified.
Third offense: A parent meeting will be scheduled to review program placement.

Severe misconduct:

**Regardless of whether the offense is a first-time offense, I may be removed from the course and or program with a grade of failing and will fall within the jurisdiction of reassignment to a face-to-face course at CHS.

Student Acknowledgment and Understanding:

I have read, understand, and acknowledge all the expectations and the policy set forth in this document.
I agreed to abide by the guidelines as stated.

I have read and understand the above student policy and stated expectations for my child and agree to support the online learning course expectations.
I understand that participation in my child's education will help determine his/her likelihood of success in the program.
I will monitor and support my child in his/her studies.
I will encourage my child to communicate with the teacher whenever he/she has a question or a problem.
I agree to be accessible and readily available to the online teacher to discuss my child's progress.
I understand time management and attendance is vital to my child's success.
I understand acceptance into this program may be based on class size and my child must fulfill the program expectations in order to remain enrolled.
I grant permission for my child to leave Concord High School during the online portion of their day and walk to/from the MOVE UP Academy, located in the basement of the Concord Intermediate School.
Failure to follow these policies may result in dismissal from the program.
