Reimbursements:
Forms must be signed and can be returned to Dawn Jones (jones.2272@osu.edu), preferably before the start of the conference.
Air travel must be booked on a US flag carrier.
Everyone should fill out and sign the Supplier Form. The address must be where you can receive a paper check.
US Non-residents must fill out the W8-BEN form. We will also need a copy of your passport and visa.
If you have a W or B visa, you must fill out the Compliance form.
If you are not doing a round trip between Columbus and your home institution, or if you are driving, you will also need to fill out a travel comparison form. If you are driving, we will need to know ahead of time; you may be required to use a rental car from certain providers (as opposed to being reimbursed for mileage on a personal vehicle).
Getting Here:
Plane: if arriving by airplane, the airport is John Glenn Columbus International Airport (CMH).
Car: if arriving by car, you can park in either:
Tuttle Garage: 2000 Tuttle Park Pl, Columbus, OH 43210. It is one block from Cockins Hall, where the conference will take place.
Lane Garage: 2100 Neil Ave, Columbus, OH 43210.
Take a ticket when you enter the garage. If you have contacted the organizers about travel funding, we will have a parking pass for you.
Accommodation:
There are many hotels near campus at which you can make a reservation, including:
SpringHill Suites Columbus OSU on Olentangy River Rd (30 minutes walk, shuttle service available)
The Blackwell (on campus)
There are also more hotel options near by if one of these does not work.
