2020-2021 Required Documents

Paw Creek Parents,

We have two vital documents that we need you to complete:

• The 2020-2021 Student Code of Conduct is available online here. On page 3 of the Student Code of Conduct students and families will need to complete the Student and Parent Acknowledgement and Pledge Document.

• Annual Military Student Identifier Collection Form. We need every parent to fill this out whether he/she is in the military or not.

The deadline to fill out these forms is Friday, September 25.

We have a few options to best meet your needs.

• Option 1- You can click the inks below to complete this process electronically.

• Option 2- You can print a copy of the two forms, sign them, and drop them off at the school in the red secure drop box.

• Option 3- You can pick up paper copies along with an envelope from the school, sign them, and drop them off at the school in the red secure drop box.
