Reporting student  Absences 

My child is unable to attend school...what should I do?! 

If your child is unable to attend school for any reason, please complete the form below. This mandatory form will be submitted to the school. 

Please be sure to: 

• Include the dates your child missed school 

• The reason for the absence 

• Upload any needed documentation (Doctor's notes, etc.) 

ALL parents/guardians will need to submit this online form each time their child misses school. 

**Educational field trips will no longer be accepted. If your family plans on taking a trip, the absence will be UNEXCUSED**
