To ensure your student is successfully enrolled and placed in the appropriate courses, please bring the following required documents:

• Student Enrollment Form 

  • CMS Enrollment Form - Eng or 

  • CMS Enrollment Form Spanish

• Proof of Date of Birth and Legal Name (see guidelines below)

• Parent ID

• Proof of Residency (see guidelines below)

• Safe Schools Declaration

• Current Immunization Record/Health Assessment

• Transcript from Previous Academic Year

All children entering NC public schools for the first time must submit proof of a Health Assessment.*

*These documents are required upon student entry. A “30 calendar day grace period” for submission is granted for students not having required documentation.

Important: 

• Families with complete documents will be processed most quickly and can expect to move through the process smoothly.  

• Failure to bring any of the required documents will delay your student's enrollment until all documentation is received. Please refer to the attached checklist to ensure you have everything needed before your arrival.
